Our Tax Exemption form is accepted at most retailers. As a 501(c)(3), we should not pay sales tax. Sam's Club, Walmart, Costco and Amazon prove to be a little difficult. We are working on an Amazon account for PTA as well as creating the appropriate accounts with the retailers mentioned.
Office Depot account is complete and will mark your purchases as tax exempt when using the number below.
Sunnyvale PTA seeks to be good stewards of the resources the parents, students, teachers and sponsors entrust us with. When you are making purchases on behalf of the PTA, please be price conscientious. The Reimbursement form included below is necessary when requesting reimbursement or a check made directly to a vendor/retailer. It is important to give at least 3 days notice, if not more, when requesting a check. Reimbursements can be emailed to treasurer@sunnyvalepta.org or left at any campus in the PTA box.
From time to time, you may be asked to help with PTA sales. There are only a few a year, but when they happen, it is important to follow our by-laws and standing rules when handling money. The Income Collection Form will help our treasurer correctly allocate the funds to the appropriate budget line, so be sure to indicate why the money was collected (ticket sales, powder puff, spirit wear, etc.). Next, have two members count the money and sign the Income Collection Form. Then, seal the completed form and money in an envelope and deliver to the treasurer.
